Download Statutory Declaration Form
The form is required to be completed in the presence of a Justice of the Peace, solicitor or another person authorised to take a statutory declaration under section 9 of the Oaths and Declarations Act 1957.

The completed form needs to be scanned into 1 document, including a copy of an Identity Document for the person verifying your identity.
The person verifying your identity:
- must not be a relative (a relative is a person connected by blood or marriage) must not be living at same address
- must be 18 years of age or over, and
- must have known you for more than 12 months.
